Attacker opens door.  As he enters the vehicle cock your arm back ready to strike.  When he is close enough do it. As he moves backwards, drive car forward continue to hit him.

If you either have keys in your hand or can get them out of the ignition, drive them into his eye, scratch his face with them, slash across his forehead with them (causes quick bleeding that may scare him into thinking he is seriously injured (he isn't though).

Use arm bar to drive his head and face into your car.  Cup his elbow with the closest hand and hold his other hand against your body for best leverage.  Push with hand on elbow and pull with hand on you. You should make a swinging motion with your shoulder and hips.

Last Defense is a chop to the throat, continue to strike after that.   If in gear drive away.
